All clinicians affiliated with CCI are eligible to participate in the CCI Rewards program.†

CRNA: $2,500
Anesthesiologist: $4,000

CCI Anesthesia will provide no compensation for pursuing or obtaining recruitment referrals other than that set forth above. Per state and federal law, the individual will pay all taxes and fees applicable to such reward.

The following are guidelines on qualifications and exclusions for a referral payment:

The referred Provider must execute a contract for a permanent position (“Contract”) providing services at CCI client hospital or surgery center and commence providing services under the terms of the permanent Contract within one (1) year of referral;

Provider’s Contract Term must be at least THREE (3) years;

CCI must pay no agency fee for the recruited Provider.

Providers will be excluded from referral payment if:

CCI has paid an agency fee for the recruited Provider;

The Provider is transferring from one CCI site to another;

The Provider is under Contract with CCI;

The Provider is retained during CCI’s transition with a new client.

A recruitment referral will be eligible for award payment for one (1) year following the referral submission to CCI Anesthesia.†† Our recruiting team will track the referral’s status throughout the contracting process and follow up with you once all requirements are met.

External Referral Program Terms and Conditions

To receive a payment (herein “External Referral Payment”) pursuant to CCI Anesthesia (“CCI”)'s external referral program (“External Referral Program”), you must comply with the terms and conditions (“Terms”) set forth below, and you agree to all Terms as set forth below.

1. Individuals Eligible To Participate

To be eligible to participate in the External Referral Program, you must be an individual person (not a business entity) who is at least eighteen (18) years of age, a legal resident of the United States, and otherwise meet the qualifications set forth herein.

2. Individuals Ineligible To Participate

The following people/entities are ineligible to participate in the External Referral Program:

• Current employees of CCI and their immediate family members
• Employees of staffing or recruiting agencies or any other entity that CCI would be required to pay for a referral absent this External Referral Program
• Individuals who are prohibited from referring a candidate to CCI pursuant to a non-competition or non-solicitation agreement or other restrictive covenant agreement or whose referral of a candidate (in CCI's sole discretion) could expose CCI to liability

3. Ineligible Candidates

An External Referral Payment cannot be earned for referring any of the following:

• Individuals who have already applied to CCI within the past twelve (12) months
• Individuals who have been a contractor, temporary worker, consultant, or provided any service to CCI as an employee of a vendor or subcontractor within the past twelve (12) months
• Individuals who are currently working on a contract with CCI
• Individuals who have restrictive covenants, such as non-competition or non-solicitation agreements or any other agreement that, in CCI’s sole discretion, could prevent CCI from contracting with them
• Candidates who have already been referred to CCI by a CCI employee or an external referent; in the event that more than one person refers a candidate, the person who refers first and otherwise qualifies will receive the payment
• Self-referral
• Former CCI employees
• Family member of a current CCI employee

4. Payment Terms

The External Referral Program offers a one-time referral payment on the eligible and successful referral of a person unaffiliated with CCI in order to attract, hire and retain the best talent in the United States, provided that you are an eligible individual and refer an eligible candidate to one of the positions of the identified campaign, and the following takes place:

CCI successfully contracts with the individual (or a professional entity wholly owned by such individual) you referred for one of the eligible positions. CCI will begin the process to payout your External Referral Payment to you upon the new contractor’s first day of service.

• You return a completed IRS Form W-9 as required
• You complete the External Referral Profile Form and otherwise comply with all Terms of this External Referral Program
• The candidate that you referred acknowledges that you referred him or her prior to the candidate’s acceptance of an offer

With respect to any payment you receive pursuant to this External Referral Program, you agree that you are solely responsible for paying appropriate state and federal taxes.

PROCESS TO REFER CANDIDATES

• Participants can pass referrals by completing the referral form on the Program webpage or contacting Christie Buchholz, Director of Staffing and Recruitment, at 251.625.6995 or cbuchholz@ccia.health. Referrals submitted any other way that do not include all of the required information or that are not properly identified will NOT be eligible for the External Referral Program.
• To be eligible for the External Referral Payment, you must provide the following at the time of your initial submission:
• Your full name and email address
• The potential candidates’ contact information to include: full name, city, state, and email address
• The potential candidate’s current job title
• The potential candidate’s resume (and other documents, if applicable)

5. General

• Participation in the External Referral Program is voluntary and does not create an employment or contractual relationship between you and CCI.
• You agree to comply with all applicable laws and regulations while participating in the Program.
• These Terms shall be governed by and construed in accordance with the laws of each jurisdiction in which CCI operates.
• These Terms constitute the entire agreement between CCI and you regarding the External Referral Program and supersede any prior agreements or understandings, whether oral or written.
• If any provision of these Terms is found to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable to the fullest extent permitted by law.
• You agree that CCI’s total liability related to the External Referral Program is your actual damages or $4,000, whichever is less.
• Any External Referral Payment will be paid to you within thirty (30) days after the completion of all requirements to make you eligible to receive the External Referral Payment.
