Use the following guidelines to qualify your lead:

• Provide specific information or an introduction to a person of influence at a potential client hospital.
• Inform CCI Anesthesia that a hospital is seriously considering making a change with regard to full-time anesthesia practice management services.
• Share “intelligence” that a hospital is experiencing significant instability with its current service (something not commonly known but is not considered confidential or proprietary) such as:
  • A hospital putting an anesthesia service contract out to bid
  • A recruiter mentions an anesthesia opening at another hospital
  • A colleague looking to leave their current hospital
  • A colleague taking a new job, leaving a vacancy behind
  • A conference attendee expressing concerns about their hospital’s anesthesia services
  • Reports of service issues at a hospital within a health system
  • Rumors of anesthesia service challenges at a competing hospital
  • Discussions of anesthesia service issues on blogs or forums

Providers will be excluded from lead payment if:

CCI Anesthesia is currently in discussions with the potential client hospital or surgery center.

• Leads remain eligible for six months from submission or as long as they remain active.
• Payment is made within 30 days after CCI begins services at the new hospital.
• Participants are responsible for any applicable taxes or fees.
• CCI Anesthesia reserves the right to modify or terminate this program at any time.

Terms and Conditions for CCI Rewards Lead Pass Program

To receive a payment (herein “Lead Pass Payment”) pursuant to Clinical Colleagues, Inc.’s (“CCI”) Rewards Lead Pass Program (“Rewards Lead Pass Program”), you must comply with the terms and conditions (“Terms”) set forth below, and you agree to all Terms as set forth below.

1. Individuals Eligible to Participate: To be eligible to participate in the Rewards Lead Pass Program, you must be a clinician affiliated with CCI, have a current and active contract with CCI, and cannot be directly or indirectly involved in granting the clinical services contract.
2. Individuals Ineligible to Participate: The following people/entities are ineligible to participate in the Rewards Lead Pass Program:
   • Current employees of CCI and their immediate family members
   • Employees of staffing or recruiting agencies or any other entity that CCI would be required to pay for a referral absent this Rewards Lead Pass Program
3. Individuals who are prohibited from referring a hospital or health institution pursuant to a non-competition or non-solicitation agreement or other restrictive covenant agreement or whose referral of a hospital or health institution (in CCI’s sole discretion) could expose CCI to liability.
4. Ineligible Institutions: A Rewards Lead Pass payment cannot be earned for referring a potential client hospital or health institution with which CCI is already in discussions.
5. Process to refer Institutions: Participants can pass information by contacting Nathan Yoder, Executive Vice President of Practice Improvement at 480-569-7886 or nyoder@cciah.health and provide details about the lead, including information about the lead’s current situation. Referrals submitted any other way that do not include all of the required information or that are not properly identified will NOT be eligible for the Rewards Lead Pass Program. In addition, information provided to CCI shall be in compliance with all data privacy laws and regulations, and use of spam or unsolicited email to source or gather information is strictly prohibited.
6. Eligibility of Reward: Upon receipt of all pertinent information, CCI shall determine at its sole discretion whether you are eligible for the Rewards Lead Pass Program Payment.
7. Payment Terms: The Rewards Lead Pass Program offers a one-time referral payment on the successful execution of a new practice management contract with CCI. CCI will begin the process to payout your Rewards Lead Pass Program Payment to you once CCI begins providing services at the new client hospital or health institution.
   • You must have a current and active contract with CCI at the time services begin at the new client hospital or health institution
   • You return a completed IRS Form W-9 as required
   • You complete all required information for the Rewards Lead Pass Program and otherwise comply with all Terms of this Rewards Lead Pass Program
   • With respect to any payment you receive pursuant to this Rewards Lead Pass Program, you agree that you are solely responsible for paying appropriate state and federal taxes
8. General:
   • Participation in the Rewards Lead Pass Program is entirely voluntary.
   • You agree to comply with all applicable laws and regulations while participating in the Rewards Lead Pass Program.
   • These Terms shall be governed by and construed in accordance with the laws of each jurisdiction in which CCI operates.
   • These Terms constitute the entire agreement between CCI and you regarding the Rewards Lead Pass Program and supersede any prior agreements or understanding, whether oral or written.
   • If any provision of these Terms is found to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable to the fullest extent permitted by law.
   • You agree that CCI’s total liability related to the Rewards Lead Pass Program is your actual damages or Rewards Lead Pass Program Payment, whichever is less.
   • Any Rewards Lead Pass Program payment will be paid to you within thirty (30) days after CCI begins providing services at the new client hospital or health institution provided all requirements to make you eligible to receive the Rewards Lead Pass Program Payment have been met.
